  • (ie. The URL/web address of the organization that you are choosing to nominate for Outreach Collection Recipient for the Church Year in question.)
  • (ie. Who and/or what is the focus of outreach for the organization that you are choosing to nominate as an Outreach Collection Recipient for the Church Year in question. e.g., environment, homeless, disabled, veterans, at-risk children)
  • (e.g., How many do they help per year, what effect have they had)
  • (ie. The operating budget for one year or the total revenue received for one year by the organization that you are choosing to nominate for Outreach Collection Recipient for the Church Year in question.)
  • (i.e., Use the checklist below to describe characteristics of the organization that you are choosing to nominate for Outreach Collection Recipient for the Church Year in question.)
  • (Please describe below your Outreach Collection Recipient's Primary Source of Income as you've chosen to list such previously as, "Other".)
  • (ie. List the geographical scope and/or locations of service enacted by the organization that you are choosing to nominate for Outreach Collection Recipient for the Church Year in question.)
  • If your organization is selected as a recipient, we will ask you to be the monthly sponsor:

    Information about all Nominated Organizations will be distributed to the Congregation prior to the next Congregational Meeting. At which point voting for that Church Year’s Outreach Recipients will take place.
  • Below are the volunteer commitments, you MUST agree to make upon your organization being selected. Please choose as many as you feel are appropriate to your personal schedule, as well as the needs of your Outreach Recipient.
